This new service can add to what you already know, supporting you with information and analysis that will help you make informed health decisions.
Using a detailed health questionnaire similar to the one we use in our consulting practice, we’ll evaluate your health status. We ask about a wide range of health and medical issues, then tell you what we see based on our research and on our health consultation practice.
And be assured that the information you provide is kept strictly confidential.
Completing the questinnaire consists principally of checking off the conditions that apply to you. But you also have the opportunity to provide information that’s specific to your health history.

When you purchase the service you’ll first download the health questionnaire. It's a PDF file, so you’ll need a recent version of Adobe Acrobat. If you don't already have it, click here.
The questionnaire/PDF file is specially formatted so you can complete it directly on your computer. When you’ve complete the questionnaire, you can email it back to us.
Within a week, we’ll email our evaluation report to you. For a small fee, you can have a printed copy mailed to you.
This service costs $50.
